The Medicare Annual Wellness Visit is an appointment with your primary care doctor. It's covered once a year if you have had Part B for more than 12 months.

This visit is not a head-to-toe physical. The goal is to create or update a plan to help you stay healthy. It's designed to help prevent disease and disability based on your current health and risk factors.

The goal is to create a wellness plan

As part of your Annual Wellness Visit, your doctor will ask you to fill out a questionnaire called a Health Risk Assessment. Answering these questions can help you and your doctor come up with a plan to help you stay healthy. During your visit, your health care team will:

Below are common questions and answers about Medicare and your Annual Wellness Visit.

  • The Annual Wellness Visit is a yearly appointment with your doctor to create or update a personalized health plan. The goal is to help prevent disease and disability based on your current health and risk factors.

  • Call your doctor's office and say that you want to schedule your Medicare Annual Wellness Visit.
    OR
  • Your Annual Wellness Visit is covered if:

    • You've had Medicare Part B for 12 months or more
    • You haven't had your Annual Wellness Visit in the last 12 months
    • You completed your Welcome to Medicare visit 12 months ago or more

    Have you had Part B for more than 12 months? If you didn't have a Welcome to Medicare visit, your Annual Wellness Visit is still covered.

  • You can schedule your visit as soon as you know your Medicare eligibility date. Just remember that the date of your appointment should be within the first 12 months of your eligibility date.
    OR
  • Your doctor may send you a questionnaire to fill out and bring with you to the appointment. It’s also a good idea to bring a list of any medications you’re taking. If you have any questions or concerns you’d like to talk about, write those down too.
