We offer adult and adolescent care, pediatric well child care and immunizations, gynecological exams including pap smears, biopsies and birth control, minor surgical procedures, sports and school physicals, and on site radiology and laboratory services. 

Dr. Robert Habig offers FAA flight physicals for Class 1, 2, and 3.  Please call his office at 317.867.1236 to schedule an appointment. Dr. Robert Habig has office hours M-T-Th-F from 8-4:30.

Dr. Forkin has office hours M-W-Th-F from 8-4:30.  Please call her office at 317.867.1101 to schedule an appointment.

Dr. Elaine Habig has office hours Monday and Friday and her Nurse Practitioner, Carla Erickson covers the practice on Tuesday and Wednesday. Please call her office at 317.867.1115 to schedule an appointment.
