Below are common questions and answers about Medicare and your Annual Wellness Visit.

  • The Annual Wellness Visit is a yearly appointment with your doctor to create or update a plan for your health that is right for you. We'll take your current health and any conditions into account. The goal is to keep you healthy.

  • Call your doctor's office and be sure to mention that you're scheduling your Medicare Annual Wellness Visit.

  • Your Annual Wellness Visit is covered if:

    • You've had Medicare Part B for 12 months or more
    • You haven't had your Annual Wellness Visit in the last 12 months
    • You completed your Welcome to Medicare visit 12 months ago or more

    Have you had Part B for more than 12 months? If you didn't have a Welcome to Medicare visit, your Annual Wellness Visit is still covered.

  • You can schedule your visit as soon as you know your Medicare eligibility date. Just remember that the date of your appointment should be within the first 12 months of your eligibility date.
    OR
  • Your doctor may send you a questionnaire to fill out and bring with you to the appointment. It's always a good idea to bring a list of medications you're taking.

    Write down questions to bring with you to your visit, along with any concerns you'd like to discuss.
